BENEFITS OFFERED Medical thru Medica(Passport)Employer Contribution/Cafeteria Plan Amounts:$850 Individual/$1600 Family (Monthly) HSA Employer Contribution: $50 Individual/$100 Family (Monthly) We offer two plans: (Passport Plan) Single plan HSA $753.38 with $4000 deductible 0% after deductible Single plan Non-HSA $828.62 with $1500 20-25% after deductible Family plan HSA $1838.46 with $4000 deductible 0% after deductible Family plan Non-HSA $2071.58 with $1500 20-25% after deductibleELIGIBILITYYou and your dependents are eligible for Western Prairie Human Services benefits on the first of the month following 1 month of employment.Eligible dependents include your spouse, children under age 26 or disabled dependents of any age. Elections remain until the next open enrollment unless you or your family members experience a qualifying event. If you experience a qualifying event, you must contact HR within 30 days.
