Western Prairie Human Services is proud to offer a comprehensive benefits
package to eligible, full-time employees who work 20 hours per week. The
complete benefits package is briefly summarized in this booklet. You will receive
plan booklets, which give you more detailed information about each of these
programs.

BENEFITS OFFERED
Medical thru Medica(Passport)
Employer Contribution/Cafeteria Plan Amounts:$850 Individual/$1600 Family (Monthly)
HSA Employer Contribution: $50 Individual/$100 Family (Monthly)
We offer two plans: (Passport Plan)
Single plan HSA $753.38 with $4000 deductible 0% after deductible
Single plan Non-HSA $828.62 with $1500 20-25% after deductible
Family plan HSA $1838.46 with $4000 deductible 0% after deductible
Family plan Non-HSA $2071.58 with $1500 20-25% after deductible
ELIGIBILITY
You and your dependents are eligible for Western Prairie Human Services benefits on the first of the month following 1 month of employment.
Eligible dependents are your spouse, children under age 26, disabled dependents of any age, or Western Prairie Human Services eligible dependents.
Elections made now will remain until the next open enrollment unless you or your family members experience a qualifying event. If you experience a qualifying event, you must contact HR within 30 days.
